In order to keep an update medical record for your child, we would very much appreciate it if you would answer the following questions:
If your child is taking any prescription medication that has to be given during school hours and he/she is in KG1, KG2, or primary school. Kindly bring the medication YOURSELF to the School Doctor so that it can be administered to your child as required. PLEASE DO NOT SEND MEDICATION WITH YOUR YOUNG CHILD. If your child is in Secondry School, please give the medication to your child to give to the School Doctor for safe administration to him/her. Please send a written and signed note stating your child's name, class#, and the exact time and dosage of the medication to be taken. Medication may be collected from the Doctors' office by parents or by Middle/High School students
Please contact the school Clinic if you have any concerns. Please do not forget to attach any/all relevant medical records to this form.
I hereby give permission to the school clinic to administer all necessary check-ups, first aid, non-prescriptive medications, and act in casualties including admittance to the hospital in case of emergency.
